If the online form isn’t currently available it means the Practice is closed. Please see section ‘If you need help when we are closed’ for further information.

To make appointment for blood test, travel vaccine or any other Nurse appointments please call us on 01582 429953 after 3:00 pm or use our online service via our website. This will avoid you having to wait in the phone queue.

Your appointment

Once we have received your online request it will be reviewed by the Doctor who will prioritise them as needed:

  • Refer to local pharmacist, Physiotherapy, Health & Well being service etc
  • Carry out a phone consultation
  • Offer face to face consultation
  • Offer video call
  • Reply by text or email

Phone, video call or by text or email consultations offer a flexible way to get the care you need, often more quickly.

Cancelling or changing an appointment

To cancel your appointment:

  • Phone us on 01582 429953 then select Option 1 followed by Option 2 and leave a message. Kindly give us at least 3 hours’ notice so we can offer your appointment to someone else in need.
  • You can use your NHS account (through the NHS website or NHS App)
This surgery is extremely busy because of the high demand for appointments. We cannot continue to provide General Medical Services to clients who continually misuse the services.

Patients will be sent out written warnings if they miss their appointment with the doctor.

If a patient misses three appointments within a 12-month period, without a valid reason, they will be sent out a letter of removal from the practice.

Could you please cancel appointments at least three hours before your appointment time if you are unable to attend.

All appointments will be marked as ‘did not attend’ if we have not been given up to three hours’ notice.

If you need help when we are closed

If you need medical help when we are closed, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

For minor ailments, consider your local pharmacy. The pharmacy can provide advice and treatment for many common illness.

Home visits

We understand that visiting the surgery might not always be possible. While our facilities are best-equipped to provide care, we offer home visit for patients who are truly housebound.

  1. For any home visit request please call us by 11.00 am on 01582 429953

2. Be ready to provide details about your condition to help us assess your needs.

Please note: Simply not having transport doesn’t necessarily qualify for a home visit.

For additional support: If you have been referred by a GP, you may receive a visit from a Community Nurse, Community Matron, Single point of contact nurse or a PCN team member. Health visitors may visit if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
